Section 1730-05-.13 - CHANGE OF NAME AND/OR ADDRESS(1) Change of Name - An individual registered with the Board must notify the Board in writing within thirty (30) days of a name change. The notice must provide both the old and new names and must reference the individual's profession, board, and certificate number.(2) Change of Address - A licensee or certificate holder must notify the Board of a change of address within thirty (30) days of such change.
