Section 5005.4 - Subscription application requirements(a)Rejection of application. A subscription application may be rejected for any of the following reasons: (1) all information required by the commission is not provided;(2) the application includes a form of payment that is not acceptable to the commission or the form of payment is rejected;(3) the applicant is under 18 years of age;(4) the applicant's identity cannot be verified;(5) the applicant's physical location cannot be identified at time of attempted purchase;(6) the applicant's physical location is outside the State or any commission-designated prohibited sales area at time of attempted purchase; or(7) the applicant is on the commission's voluntary self-exclusion list.(b)Termination. A subscription may be terminated after acceptance if, before or after the start date, the commission is notified that the payment for the subscription is invalid (e.g., a payment returned for non-sufficient funds). In such event, the subscription shall be deemed invalid, and no prizes will be paid to the applicant relating to any period in which the subscription was invalid.N.Y. Comp. Codes R. & Regs.
