Section 30.332 - Biennial Reporting(1) A Large Quantity Generator, as described in 310 CMR 30.340(1), who transports or offers for transportation any hazardous waste off the site of generation shall prepare and submit a copy of a Biennial Report to the Commissioner by March 1st of each even-numbered year. The Biennial Report shall be submitted in compliance with 310 CMR 30.006 on EPA Form 8700-13A. The report shall cover activities during the previous calendar year and shall include at least the following information:(a) The EPA identification number, name and address of the generator;(b) The calendar year covered by the report;(c) The EPA identification number, name and address of each off-site facility in the United States to which hazardous waste was sent during the year;(d) The name and EPA identification number of the transporters used during the reporting year for shipments to an off-site facility within the United States;(e) A description, EPA hazardous waste number, DOT hazard class, and the quantity of each hazardous waste sent to an off-site facility within the United States. This information shall be listed by the EPA identification number of each such off-site facility to which waste was sent.(f) A description of the efforts undertaken during the year to reduce the volume and toxicity of waste generated.(g) A description of the changes in volume and toxicity of waste actually achieved during the year in comparison to previous years going back to 1984. This comparison shall also be made to years prior to 1984 to the extent such information is available for years prior to 1984.(h) The information described in 310 CMR 30.104(3)(b)2.g.(iii) and 310 CMR 30.104(3)(d)2.f.(iii) on the waste involved in any treatability or research studies.(i) The certification signed by the generator or authorized representative in compliance with 310 CMR 30.009.(2) Any Large Quantity Generator who treats, stores, or disposes of hazardous waste at the site of generation shall submit a Biennial Report covering those wastes in compliance with the provisions of 310 CMR 30.544.(3) Reporting for exports of hazardous waste is not required on the Biennial Report form. A separate annual report requirement is set forth at 40 CFR 262.56, as adopted at 310 CMR 30.361.(4) Reporting for waste managed in a wastewater treatment unit in compliance with 310 CMR 30.605 is not required on the Biennial Report form.(5) Reporting for all Class A and state-only regulated Class B and Class C regulated recyclable materials, managed in compliance with 310 CMR 30.200, is not required on the Biennial Report form.
