Section 120.134 - Amendment of Licenses and Registration Certificates at Request of Licensee(A) Applications for amendment of a license shall be filed in accordance with 105 CMR 120.124 and shall specify the respects in which the licensee desires the license to be amended and the grounds for such amendment. Applications for amendment of sealed source and device registration certificates must be filed in accordance with 105 CMR 120.128(N) and any other applicable provisions and must specify the respects in which the certificate holder desires its certificate to be amended and the grounds for the amendment.(B) An invoice for an amendment fee will be issued on receipt of a request to amend a license. The amendment will not be issued until after the invoiced amount has been paid.Amended by Mass Register Issue 1373, eff. 9/7/2018.
