Section 3854.1 - Mental Health Loan Assumption Application(a) The loan assumption application shall include, at a minimum: (1) Applicant's name and social security number.(2) Applicant's address and telephone number(s).(3) Applicant's date of birth.(4) Name of educational lending institution(s).(5) Balance(s) of the applicant's eligible educational loan(s).Cal.
