Section 203 - Master Certification for Each FicheA master certification in substantially the form set forth in subdivision (a) shall be completed for each fiche which is prepared for records maintained by State agencies.
(a) Master Fiche Certification (1) The images appearing on this fiche are made in accordance with the authority granted to [name of agency and specific organizational unit] pursuant to [citation to authority].(2) It is hereby certified that the records which are to be reproduced on this fiche are those records which have been kept or recorded by the [agency and unit] in the regular course of business.(3) These records are being delivered into my possession as legal custodian for the filming of micrographic images.(4) The microfiche images recorded hereon are accurate and complete reproductions of the original records. (Where the agency is filming copies of original documents, it shall state the location of the originals, omit subdivision (a)(5), and revise the cross-referenced certification in subdivision (b) accordingly.) These images are made by the camera operator whose name appears on the margin of each microfiche image which is reproduced.(5) The original records reproduced hereon were destroyed in the regular course of business in accordance with [cite authority] and pursuant to procedures established by the [agency and unit].(6) It is further certified that the following original correspondence, records, or other papers of the [agency and unit] are reproduced on this microfilm file: ___________________________
