Section 448.807 - Application for license(a) Submission to department.--Any person desiring to secure a license to maintain and operate a health care facility shall submit an application therefor to the department upon forms prepared and furnished by it, containing such information as the department considers necessary to determine that the health care provider and the health care facility meet the requirements of licensure under the provisions of this act and the rules and regulations relating to licensure. Application for renewal of a license shall be made upon forms prepared and furnished by the department in accordance with the rules and regulations of the department.(b) Fees.--Application for a license or for renewal of a license shall be accompanied by the following fees: (1) | Regular or Special License: |

(b.1)Licensure Fee Account.--(1) The Licensure Fee Account is established as a restricted account in the General Fund.(2) Fees under subsection (b)(1) pertaining to abortion facilities shall be deposited in the account.(3) Money in the account is appropriated on a continuing basis to the department for use in the performance of its duties.(c) Bond.--The department by regulations may require new applicants for a license to post a bond.1979, July 19, P.L. 130, No. 48, § 807, added 1980, July 12, P.L. 655, No. 136, § 7, imd. effective.
