Section 465 IAC 2-2-2 - Application for license for child-placing agency

Sec. 2.

Application for License for Child-Placing Agency. Application for a license to operate a child-placing agency shall be submitted to the State Department of Public Welfare on the forms provided for that purpose by the State Department of Public Welfare.
