Section 35-2-903 - Application for license; submission of evidence prerequisite to issuance(a) An applicant for a license under this act shall file a sworn application with the division on a form provided by the division. The form shall request the following information: (i) The applicant's name; (ii) The type of health care facility to be operated; (iii) A description of and the location of the facility buildings; (iv) The name of the person in charge of the health care facility; (v) Whether the applicant has had a license to operate a health care facility or agency providing health care services in this or any other state denied, suspended, revoked or otherwise terminated for cause and the specific reasons for such action. Evidence that the facility subject to the application is currently in compliance with all applicable statutes, rules and regulations is required; (vi) Evidence that the applicant is capable of complying with applicable rules and regulations; (vii) Such other information as the division may require pursuant to rules promulgated under this act. (b) An application by other than an individual shall be made by two (2) officers of the organization or by its managing agents.
