Every application for license or license renewal must be accompanied by:
(1) A certificate to the effect that the economic poisons that he intends to use or is using are registered in the Department of Agriculture pursuant to the provisions of §§ 1001—1013 of this title.
(2) Proof accrediting that those who are to supervise or perform the application of insecticides or economic poisons have experience, ability and skill in the application of such insecticides or economic poisons.
(3) Proof of having taken insurance, to be effective when the license is issued, from an insurance company duly authorized to do business in Puerto Rico, in an amount not less than fifty thousand dollars ($50,000) for one person, or one hundred thousand dollars ($100,000) for one occurrence, to answer for any damages which, by action or omission, might be caused to another through guilt or negligence.
The Office of the Insurance Commissioner shall approve said policy as to form and sufficiency. An adequate book for registering such policies shall be kept at the Department of Health, which register shall be available for examination by any interested person.
Insurance companies issuing insurance policies pursuant to this section must notify the Secretary of Health of the cancellation or nonrenewal of the policies required by this chapter.
(4) Payment of the fees hereby established.
