Any petition for review or proceeding brought against a decision of the Electoral Review Board within a period of thirty (30) days prior to an election shall contain the following numbered parts, in the same order provided herein:
(a) Citation of the legal provisions which justify the invocation of the jurisdiction of the court;
(b) a brief summary of the facts pertinent to the review;
(c) a short and concise statement on the questions of law raised, and
(d) the argument of the questions raised.
The cover of the review shall only bear the caption (which shall identify the petitioner as such and the adverse parties as defendants), and the name, address, and telephone number of the attorney for petitioner and for the adverse parties. Immediately following, there shall be a detailed index which shall meet as far as possible the requirements of Rule 38 of the Rules of this Court, App. I-A of this title. Any document which should be brought before the Court’s consideration at this stage of the proceedings shall be annexed at the end of the petition as an appendix to the same. The petition shall not have more than ten (10) pages, excluding index and appendix.
