Section 27-1-1.5-33 - "Uniform application for third party administrator license"

"Uniform application for third party administrator license" refers to the document entitled "Uniform Application for Third Party Administrator License" that is:

(1) published by the NAIC; and

(2) in effect on January 1, 2018.
