Section 25-8-4-14 - Duplicate license; issuance procedure

The board may issue a duplicate license to a person licensed under this article if the person:

(1) files a verified statement with the board that the original license has been lost or destroyed; and

(2) pays the fee required under IC 25-1-8-2.
