Section 32-3426 - Renewal of license; inactive status; notice of address or name changeA. Except as provided in section 32-4301, a license issued under this chapter is subject to renewal every two years and expires unless renewed. The board may reinstate an expired license if the licensee: 1. Complies with board rules for renewal of licenses.2. Is not in violation of this chapter or board rules or orders.3. Pays the fees prescribed pursuant to section 32-3427.B. A licensee may request and the board may grant inactive status to a licensee who ceases to practice as an occupational therapist or occupational therapy assistant.C. The board may establish by rule additional requirements for license renewal to require the successful completion of a prescribed number of hours of continuing education as a condition of licensure renewal.D. A licensee must report to the board in writing a name change and any change in business or home address within thirty days after the change.
