These forms were created in 1994 by the Alaska Supreme Courts Advisory Committee on the Probate Rules. The forms were updated in 2003 and 2008 to reflect statutory changes.
The forms are provided for illustration only. They have not been reviewed or approved by any court and should only be used after consulting the appropriate statute or rule.
