Section 13-3 - Personnel Change-In-Status Form

Changes in employment status by a peace officer, correctional officer, detention officer, or dispatcher shall be made by completing applicable parts of the Personnel Change-In-Status Form. (This form shall be submitted to the Commission within fifteen (15) days of change in status.) W.S. 9-1-704(a) and W.S. W.S. 9-1-710(a) This form should also be used to report name changes.
