Section 388-823-0025 - Who may apply for a DDA eligibility determination?(1) You may apply for a DDA eligibility determination for yourself.(2) A person may apply for a DDA eligibility determination on your behalf if the person is:(a) Delegated to consent to routine medical care for you under WAC 110-148-1560;(b) Your parent if you are under 18;(c) Your caretaker relative under WAC 182-500-0020;(e) Your authorized representative under WAC 182-503-0130;(f) Applying for you because a medical condition prevents you from applying; or(g) Someone to whom you or the courts have given permission to apply on your behalf.(3) If you or your authorized representative request it, DDA will withdraw your eligibility application or terminate your eligibility.Wash.
