(a) Any person wishing to inspect or copy public records of the authority should make the request in writing on the authority's request form or through an online portal, or by letter, fax, or email addressed to the public records officer at the email address publicly designated by the authority, or by submitting the request in person at Washington health care facilities authority at 410 11th Avenue S.E., Suite 201, Olympia, Washington 98501 and include the following information: * Name of requestor;
* Address of requestor;
* Other contact information, including telephone number and any email address;
* Identification of the public records adequate for the public records officer or designee to locate the records; and
* The date and time of day of the request.
