Section 246-919-915 - Written agreement for treatment-Chronic pain The physician shall use a written agreement that outlines the patient's responsibilities for opioid therapy. This written agreement for treatment must include the following provisions:
(1) The patient's agreement to provide samples for biological specimen testing when requested by the physician;(2) The patient's agreement to take medications at the dose and frequency prescribed with a specific protocol for lost prescriptions and early refills;(3) Reasons for which opioid therapy may be discontinued;(4) The requirement that all opioid prescriptions for chronic pain are provided by a single prescriber or a single clinic, except as provided in WAC 246-919-965 for episodic care;(5) The requirement that all opioid prescriptions for chronic pain are to be dispensed by a single pharmacy or pharmacy system whenever possible;(6) The patient's agreement to not abuse alcohol or use other medically unauthorized substances;(7) A violation of the agreement may result in a tapering or discontinuation of the prescription; and(8) The patient's responsibility to safeguard all medications and keep them in a secure location.Wash.
