Section 246-101-115 - Content of case reports-Health care providers and health care facilities(1) health care providers and health care facilities shall provide the following information in each case report: (a) Patient 's first and last name;(b) Patient 's physical address including zip code;(c) Patient 's date of birth; (e) Patient's ethnicity, as required in WAC 246-101-011(4);(f) Patient's race, as required in WAC 246-101-011(5);(g) Patient's preferred language, as required in WAC 246-101-011(6);(h) For hepatitis B acute or chronic infection case reports, pregnancy status (pregnant/not pregnant/unknown) of patients twelve to fifty years of age;(i) Patient's best contact telephone number;(j) Name of the principal health care provider;(k) Telephone number of the principal health care provider;(l) Address where patient received care;(m) Name of the person providing the report;(n) Telephone number of the person providing the report;(o) Diagnosis or suspected diagnosis of the condition; and(p) Pertinent laboratory results, if avail-able. (2) Both the local health officer and the state health officer may request additional information of epidemiological or public health value when conducting a case investigation or to otherwise prevent and control a specific notifiable condition.Wash.
