Section R911-2-800 - Emergency Medical Service Dispatch Center Designation ApplicationsAn emergency medical service dispatch center shall provide:
(1) name of the organization and its principles;(2) name of the person or organization financially responsible for the service provided by the designee and documentation from that entity accepting responsibility;(3) if the applicant is privately owned, they shall submit certified copies of the document creating the entity;(4) a description of the geographical area of service; and(5) a demonstrated need for the service.Utah Admin.
