Section R156-37-607 - Surrender of Suspended or Revoked License(1) A licensee whose license has been restricted, suspended, or revoked shall surrender the license to the Division within 30 days of the effective date of the order.(2) The Division shall consider compliance with this section in evaluating an application for relicensing.Utah Admin.
