Section 361.8 - Forms and MaterialsThe Board incorporates by reference any rules that may be contained in the following forms and requires the use of these forms in doing business with the agency:
(1) Applications for Examination, Registration and Endorsements;(2) Application for Responsible Master Plumber;(3) Employer's Certification;(5) Applications for renewals of Licenses, Registrations and Endorsements;(6) Military Service Member, Veteran, or Military Spouse Supplemental Information;(7) Supplemental Criminal History Information;(8) Application for Nonstandard Testing Accommodations including the Physician or Licensed Health Care Provider;(9) Certificate of Insurance; and(10) Transfer of License Affidavit.22 Tex.
