Section 115.12 - General Application Requirements(a) Unless otherwise indicated, all applications, required information, and documentation of credentials must be submitted on department-approved forms and in the manner prescribed by the department.(b) Applicants must submit original or certified copies of documents if requested by the department.16 Tex.
