Section 351.21 - Where to Send a Request for Correction of Personal Information(a) Individuals must send requests for corrections of personal information to the Health and Human Services Commission (HHSC) division that originally collected the information in question.(b) Individuals who do not know where to send a request should send it to HHSC's Records Manager/Coordinator at 4900 North Lamar Boulevard, 4th Floor, Austin, Texas 78751-2316.1 Tex.
