Section 1180-05-.08 - RENEWAL OF LICENSE(1) Renewal application.(a) The due date for license renewal is the expiration date indicated on the licensee's initial license.(b) Method of Renewal - License holders will have a renewal application form mailed to them at the last address provided by them to the Committee. Failure to receive such notification does not relieve the individual of the responsibility of timely meeting all requirements for renewal.(c) A license issued pursuant to these rules is renewable by the expiration date. To be eligible for renewal the licensee must submit the following to the Division on or before the expiration date of the license: 1. A completed and signed renewal application form;2. The renewal fee and state regulatory fee as provided in rule 1180-05-.06; and3. Proof of current national certification by B.A.C.B.4. Proof of completing three (3) continuing education hours on cultural diversity as provided in rule 1180-05-.12(2).(d) Licensees who fail to comply with the requirements of this rule shall have their licenses processed pursuant to rule 1200-10-01-.10.(2) Reinstatement of an Expired License. (a) Licenses that have expired may be reinstated upon meeting the following conditions: 1. Payment of the renewal fee;2. Payment of the late renewal, reinstatement, and state regulatory fees as provided in rule 1180-05-.06; and3. Proof of current national certification by B.A.C.B.4. Proof of completing three (3) continuing education hours on cultural diversity as provided in rule 1180-05-.12(2).(b) Renewal application decisions pursuant to this rule may be made administratively, upon review by the Committee Administrator.(c) Anyone submitting a signed renewal form or letter which is found to be false may be subjected to disciplinary action as provided in T.C.A. § 63-11-303 and rule 1180-01.14.Tenn.
