Section 0100-08-.03 - APPLICATION FOR SERVER PERMITS(1) Every person seeking a server permit, authorizing that person to sell, serve, or handle alcoholic beverages for consumption on-premises, must apply for a permit in a manner specified by the Commission. A twenty dollar ($20.00) non-refundable application fee is required at the time of application. A cost adjustment factor for permit fees will be updated by the Executive Director of the Commission based upon the consumer price index at regular intervals beginning in January, 2013.(2) All information submitted in and with the application must be accurate and is submitted under the penalty of perjury. Failure to submit accurate information or documentation is grounds for suspension or revocation of the server permit.(3) Every complete application for a server permit includes verification from a certified program instructor that the applicant successfully completed a certified course on alcohol awareness within one (1) year of the date of the application. (a) It is the applicant's duty to ensure that the alcohol awareness program attended is certified by the Commission. To ensure the program is certified, the applicant may look for the following: 1. Conspicuous posting of Commission certification license; and2. All Certified Training Programs shall make the following statement on the cover sheet of its printed training materials: "This program has been certified by the Tennessee Alcoholic Beverage Commission as currently satisfying the requirements of the Alcohol Server Responsibility and Training Act of 1995."Tenn.
