Section 20:14:15:02 - Experience training program application

A candidate for the experience training program shall apply in writing on a form provided by the department or institution administering the program. The application must require the following candidate information:

(1) Name;

(2) Current business or home address, or both;

(3) Email address and telephone number;

(4) Driver's license number and state of issuance;

(5) Appraiser education completed;

(6) Type of appraiser credential pursued;

(7) Area of the state in which candidate intends to practice real estate appraisal;

(8) College-level education completed, if any; and

(9) A sworn declaration signed by the candidate.
