Section 12:07:20:06 - Contents of application

The stock purchase guaranty application form shall include the following:

(1) The requested amount of the loan;

(2) The purpose;

(3) Additional documentation, including the following information:

(a) A signed financial statement completed no more than 30 days prior to application;

(b) A repayment plan;

(c) Tax forms 1040 and 1040F for the two most recent tax years;

(d) A collateral summary;

(e) Other documents and information as requested by the South Dakota Department of Agriculture;

(4) The lender's signature and the date of signature; and

(5) The applicant's signature and the date of signature.
