Section 410-125-0000 - Determining When the Patient Has Medical Assistance(1) The Medical Card gives the client's name as listed with the Oregon Health Plan (OHP) and their alpha-numeric prime number.(2) Eligibility may change on a monthly basis. In some instances, eligibility will change during the month. Eligibility should be verified each time services are provided in order to assure that the client is eligible for date(s) of service.
