Section 164-1-03 - Districts; committee members(A) Each district committee shall provide the director with the following information: (1) The name, official title, business mailing address, and business telephone number of an individual residing in the district designated by the district to serve as liaison between the district and the PWC and to receive oral or written communications from the director or the administrator; (2) A roster of all members of the district's various committees, including the following information pertaining to each such member: (a) Mailing address and telephone number; (b) Name of committee or committees of membership; (c) Appointment status under division (A) of section 164.04 of the Revised Code; (d) Commencement and expiration dates of term of appointment established in accordance with division (B) of section 164.04 of the Revised Code. (B) Each district committee immediately shall inform the director of any subsequent modification to the information provided under paragraph (A) of this rule. Ohio Admin. Code 164-1-03
