Section 75-04-01-03 - [Effective 1/1/2025] Application1. An applicant shall submit an application for a license to provide services or operate a facility to the department in the form and manner prescribed by the department.2. An application is not complete until all required information and verifications are submitted to the department. The department may declare an application withdrawn if an applicant fails to submit all required information and verifications within thirty days of the department's notification to the applicant the application is incomplete.3. Within sixty days from the date of the receipt of the completed application, the department shall notify the applicant of the department's intent to grant or deny a license.N.D.
