Section 7405.9 - Recordkeeping

Each secure facility shall maintain a disciplinary record which shall include:

(a) the rule violation report;

(b) the investigation report, if prepared;

(c) the formal disposition;

(d) the sanction(s) imposed, if any; and

(e) the appeal documents.
