The determination as to the necessity for any motorist to obtain a restricted license shall be made by the commissioner based upon information provided by the motorist in a form provided by the commissioner. The applicant must make a positive statement of need. The commissioner may investigate such statement or require the verification of the facts before accepting such a statement.
