Section 810.6 - Application procedure(a) Submission of applications. Applications must be submitted in a format and in accordance with instructions prescribed by the department.(b) Information required. The information provided by the applicant shall include the following: (1) The nature, duration and purpose of the project for which the application is filed.(2) The names and qualifications of the personnel, including the project director, if any, responsible for the proposed project.(3) A description of the facilities, staff, support services and other organizational resources available to carry out the project.(4) The intended number of participants in the project, and the process by which they were identified and selected.(5) A description of the plan for evaluating the proposed project.(6) A budget for the proposed project and a justification of the amount of grant funds requested.(7) Analysis of the impact of the project, including its significance and/or importance from an occupational safety and health point of view.(8) Any other pertinent information the Hazard Abatement Board and the department may require to evaluate the proposed project.(c) The application must be submitted in triplicate to the address below: Mr. Robert Ratajack, Secretary

(d) Notice of action on application. Each applicant will receive formal notification by the board concerning the disposition of its grant application. Action taken on the grant may include, but shall not be limited to, the following: award of grant, denial of grant, request for additional information, request for modification of project proposal and notification that the application is being held for consideration due to its untimely filing.(e) Resubmission. Applications which have been previously submitted and denied may be resubmitted only if accompanied by a statement of modification of the original proposal, on a form prescribed by the department, except that applications denied solely because of lack of program funds may be resubmitted without such statement. This statement shall detail additions and/or deletions to the previous project description.N.Y. Comp. Codes R. & Regs. Tit. 12 § 810.6
