Section 425.2 - Application(a) Prior to operation of an adult day health care program, the proposed operator must apply for and receive Department approval in accordance with Part 710 of this Chapter. Such application must include a description of the proposed program, including but not limited to: (1) the need for the program, including a statement on the philosophy and objectives of the program;(2) the range of services to be provided;(3) the method(s) of delivery of services;(4) physical space to be utilized and planned use thereof;(5) number and expected characteristics of registrants to be served;(6) a description of a typical registrant's program;(7) personnel to be employed in the program, including qualifications;(8) intended use of and coordination with existing community resources;(9) financial policies and procedures;(11) methods for program evaluation; and(12) proximity to an identified number of potential registrants.(b) A residential health care facility operator that has been approved by the Department to operate an adult day health care program at its primary site may provide adult day health care services at an extension site only when such use of an extension site has first been approved by the Department under the provisions of Part 710 of this Chapter.N.Y.
