Section 16.13.15.10 - DUPLICATE LICENSEA request for a duplicate license must be made in writing and must be accompanied by the required duplicate license fee(s) as specified in Subsections K and L of 16.13.2.8 NMAC.
A. A request for a duplicate license with a legal name change must be accompanied by legal proof of the name change.B. Individuals who do not wish to request a duplicate license must continue to use the name as it appears in the Board's records in conjunction with his/her NHA professional licensure until the requirements for name change have been met.N.M.
