Section 18:3-6.5 - Uniform invoices; information

(a) All invoices, credit memoranda, delivery and receiving slips shall have the name and address of the licensee, an individual series of numbers or letters, and each series shall be numbered consecutively.

(b) All invoices, credit memoranda, and delivery and receiving slips must show:

1. The license number and the actual name and address of the licensee (and the trade name, if any) to whom the alcoholic beverages were sold or delivered; or

2. From whom they were received;

3. The date of the transaction;

4. The name and kind of alcoholic beverages;

5. Exact size of the container and the number of containers per package;

6. The number of cases, barrels, or other units; and

7. The exact gallonage of each line item listed or the gross gallonage of each class of alcoholic beverage.
