Where transfers are made from one transporter to another transporter in the course of delivery, each transporter is to set forth on his or her waybill, delivery ticket, invoice, and the proper schedule of his or her report, the name, address, and license number of the transporter to or from whom the transfer was made, the name, address, and license number of the original consignor and the ultimate consignee, and the location at which the transfer was made, in addition to the other detail required by the schedule.
