In Exhibit A title, inserted "(SHORT FORM)"; in SECTION II, paragraph 2, substituted "each conviction" for "the conviction" and ", including the circumstances under which each offense occurred, and your age at the time each offense was committed. In addition, specify" for "and"; and in SECTION IV, added paragraphs 6 through 8.
