Section Pos 1601.07 - Amount(a) Awards shall be made in the following amounts: (1) 10% upon entering practice; and(2) Year 2 through year 10 - $10,000 per year.(b) No recipient shall receive more than $100,000 in repayments.(c) The applicant shall complete an alternate W-9 form to permit payment processing.
