Section Phy 403.01 - Licenses Subject to Reinstatement(a) The following licenses shall be subject to reinstatement by the board: (1) Licenses suspended pursuant to Phy 402.08(f)(2);(2) Licenses suspended for disciplinary reasons as part of a settlement pursuant to Ahp 214 or as part of an order of the board; and(3) Licenses lapsed pursuant to Phy 402.02(b).(b) Reinstated licenses shall be: (1) Fully reinstated, under the circumstances set forth in Phy 403.02, Phy 403.03, Phy 403.04 and Phy 403.05; or(2)Conditionally reinstated, under the circumstances set forth in Phy 403.06.N.H.
