Section Lot 204.03 - Form of Complaints Complaints filed with the commission shall contain:
(a) The name and address of the complainant;(b) The name and address of the complainant's representative, if any;(c) A concise statement of the facts which caused the complainant to file the complaint;(d) The identification of any statutes, rules, orders or other authority which the complainant alleges have been violated, if known to the complainant; and(e) The name and address of the person or organization against whom the complainant complains.N.H.
