Section 641D.Sec. 41 - NEW1. Each licensee or registrant shall file with the Board:(a) His or her contact information, including, without limitation, his or her current residential address;(b) The name of his or her employer; and(c) The address of his or her place of employment.2. A licensee or registrant shall notify the Board of any change in his or her residential address within 30 days after the date of the change.3. If the Board is required to deliver any notice by mail to a licensee or registrant, the Board will mail the notice to the last residential address of the licensee or registrant which was filed with the Board.Nev.
