In addition to any information or material required by statute, a form for an application for a license as a government applicator to apply or supervise the application of a general-use pesticide will be furnished by the Director and must include:
1. The applicant's legal name, home mailing address, home street address, if different from the home mailing address, and home telephone number.2. The governmental agency for whom the applicant intends to work and the mailing address, physical street address and business telephone number of the agency.3. The type of license for which the applicant is applying.4. A listing and description of any other license or certificate held in another state by the applicant.5. Any category in which the applicant wishes to be licensed.6. The date of birth of the applicant.Nev. Admin. Code § 555.Sec. 6
