Section 453A.340 - Registration cards: Requirements for changing name or addressTo make a change to the name or address on a medical marijuana establishment agent registration card, the medical marijuana establishment agent must submit to the Division a request for the change, which must include:
1. The name on and the number of the current medical marijuana establishment agent registration card of the cardholder;2. The new name or address of the cardholder;3. The effective date of the new name or address of the cardholder;4. For a change of the address of the cardholder, the county and state in which the new address is located; and5. For a change of the name of the cardholder, a copy of any valid government-issued identification card of the cardholder which includes a photograph of the person and the new name and address of the cardholder.Nev.
