Section 20 CSR 2150-8.120 - Changes of Name or AddressPURPOSE: This rule sets out the responsibilities and procedures for name and address changes.
(1) The licensee shall notify the board of changes in name or business address within fifteen (15) days of such change(s).(2) Notification of address changes shall be made in writing including the name, mailing address and zip code and be mailed to the Board of Healing Arts.(3) Notification of name changes must be mailed to the Board of Healing Arts and shall include a copy of a marriage certificate or court decree evidencing such change.
