Section 20 CSR 2150-8.090 - Provisional Licensure ReapplicationPURPOSE: This rule provides information regarding reapplication for a provisional license for an applicant who has failed the examination.
(1) In the event the provisional licensee fails any portion of the examination, the provisional licensee shall surrender the provisional license to the Board of Healing Arts. Upon submission of a properly completed application and fee, the licensee may reapply for a new provisional license.(2) A person may hold a provisional license for a lifetime maximum of two (2) years.(3) The provisional licensure applicant must submit an Agreement to Supervise form signed by the applicant's primary supervising clinical perfusionist.
