Section 20 CSR 2150-8.040 - Annual RegistrationPURPOSE: This rule provides information to clinical perfusionists licensed in Missouri regarding annual registration.
(1) The annual registration fee shall be an appropriate fee established by the board. Each applicant shall register with the board upon a form furnished by the board before January 31 of the registration year.(2) The license must be renewed every year.(3) The renewal date of a license shall be January 31 of the registration year.(4) Each licensee is responsible for renewing the license before the expiration date and shall not be excused from paying additional fees or penalties. Failure to receive the notification form prior to the expiration date of the license shall not excuse failure to file for renewal.
