Section 20 CSR 2150-4.056 - Applicants for Provisional Licensure RenewalPURPOSE: This rule provides the requirements for speech-language pathology and audiology provisional licensure renewal pursuant to section 345.022, RSMo.
(1) Upon submission of a properly completed renewal application and fee, a provisional license may be renewed, only once, making a provisional license valid for a maximum lifetime of two (2) years.(2) The fee for provisional licensure shall be an appropriate fee, to be established by the board.(3) Applicants for provisional licensure renewal must submit the following documentation if not provided with initial application: (A) Proof of receipt of a master's or doctoral degree from an institution accredited by the American Speech-Language-Hearing Association accrediting body, in the area for which licensure is sought. Documentation shall be provided on the board-prescribed form and submitted directly to the board from the program director of the accredited institution;(B) Proof of passage of the National Examination in Speech-Language Pathology and/or Audiology. Examination scores must be submitted to the board directly from the Educational Testing Service. The passing score shall remain consistent with the passing score set by the American Speech-Language-Hearing Association, on the date of licensure application; and(C) Verification of licensure or registration to practice in another state or territory shall be submitted to the board directly from the licensing/registration agency.
